Programmable sim cards

Anyone know if there's a commercially available programmable sim cards that could be used in a 3G/4G modem?

I'm pondering the idea of putting a modem subdermal (thus removing reliance on external smartphone), but the problem is that with a regular sim card it'd be tied to a single network.

There are of course programmable sims around (yes, I know - answering my own question), but it seems none that can be actually configured with a regular mobile network like the apple virtual sim card.

Thoughts?
